Dr. Tyler Smith was born and raised in Independence, Kansas. He received a B.S. degree in Cellular Biology from the University of Kansas in 2009 and her Doctorate of Chiropractic from Cleveland Chiropractic in 2013. Raised in Independence, KS, Dr. Tyler was very active in tennis and basketball. His passion for sports leads him to find a job to empower and educate patients of all ages to move and feel better. He has been practicing in the Kansas City area for the past six years with the goal of one day opening his own clinic. He created the KC Form to positively impact the health and well-being of the Kansas City community. He is formally trained in Graston Technique, tapping techniques, activator, soft tissue therapy, hybrid technique, and thompson technique.

Dr. Tyler believes that chiropractic care provides essential health care services to target the root cause of the problem and empower others to take control of their health. Tyler is passionate about health and looks forward to educating and helping patients of all ages. Dr. Tyler wants you to leave his clinic feeling like “this guy cares.” As a physician in Kansas City, Dr. Tyler utilizes his network of community resources and massage therapists, physical therapists, orthopedists, running instructors, personal trainers, and yoga practitioners to best support patient needs.

While outside the office, Dr. Tyler loves health and lives an active lifestyle. Dr. Tyler teaches corporate fitness classes at Blue Cross Blue Shield of Kansas City. He has an ACE certificate in the pursuit of work. Tyler enjoys spending time outdoors with his wife, Lindsay, and almost 3-year-old son, Jackson. He enjoys working outside in his garden, home improvement projects, watching KU basketball and the Kansas City Chiefs and enjoying family time.

Dr. Clay Newberry was born and raised in St. He went on to earn his Bachelor’s Degree in Biology at Truman State University in 2012 where his fascination with the human body began and his desire to serve others led him to shadow several chiropractors in his community. After her first day watching, she knew that chiropractic was what she wanted to do. The connections and relationships that traditional healers had with their patients made the field even more interesting. After completing his undergraduate degree at Truman he went on to earn his Doctorate of Chiropractic from Logan University in St. Louis, graduated in 2016 with Magna Cum Laude. He is trained in mixed technique, Thompson technique, Activation Techniques, myofascial release, and kinesio taping.

Dr. Clay believes that chiropractic can improve the lives of many people and wants to be a part of improving the well-being of his community. She loves helping people achieve their goals and creating relationships with her patients. Too often health care feels rushed or out of your control. Dr. Clay wants every patient to understand what is going on with their condition and to be active and informed in making decisions about their health. He wants all of his patients to be listened to and believes in spending as much time as necessary addressing their concerns.

Dr. Clay has been practicing at Northland since 2016. He met his wife Emily at Truman and they have two wonderful children, Henry and Eloise. Outside of the office he is a sports fan and loves being outside with his family.

Misty has lived her entire life in the hometown of the KC Chiefs! In her early twenties, Misty was a CNA in the mother-child unit at SMMC, where her passion for caregiving grew. Having three children in less than four years, being a stay-at-home mom was her next adventure. After experiencing headaches that would not go away, she sought chiropractic as a last resort, and quickly saw the benefits of chiropractic care. Later, Misty made chiropractic care a part of her life.

Misty has worked with Dr. Tyler for over 6 years and is sure he is the best chiropractor in town! With three grown children and an empty nest, her passion is now focused on making sure patients get the best care when they visit Form Therapy.

Robin grew up in Smithville, MO. In his twenties, he joined the Kansas City Police Department, where he served for 29 years while raising his family. Robin began to see Dr. Tyler as a patient while at KCPD. Seeing the benefits of Chiropractic and Massage together, he had a plan for his next chapter.

After retiring from KCPD, Robin became a Certified Massage Therapist. Her sincere interest in helping people creates a healing and calming atmosphere within her practice. It is her hope to help clients achieve their health and healing goals. Whether you’re longing for a deeply relaxing massage or recovering from long-term injuries and trauma, He has the ability to meet you where you are on your journey. Robin has experience in Swedish, medical, and deep tissue massage.

When Robin has free time, you will most likely find him climbing, camping and traveling outdoors. As a volunteer with the Missouri Search and Rescue Division, Robin is often training or in workshops with her dogs.

Cheyenne grew up in Omaha, NE, where she also graduated from the Omaha School of Massage and Health Care. In 2015, he moved to KC to spend more time with his grandmother, who may or may not have been the recipient of a foot rub. After falling in love with KC, Cheyenne was licensed in Missouri as well and practiced here at KCMO.

Cheyenne practices massage therapy to reduce inflammation, increase circulation, reduce waste products and restore health. Cheyenne uses sports massage techniques within the running and cycling community, often volunteering her time. An avid runner and cyclist himself, he experiences and understands many symptoms that develop over time.

Today, Cheyenne practices yoga, meditation and bodywork as part of her routine. Being outside exercising by cycling and riding is what he enjoys the most. Working with so many like-minded people in the KC community has been fun and she hopes to continue her passion for the healing arts for many years to come.

Jennifer graduated from Eastern Oregon University as a three-time All-American with a Bachelor’s Degree in Physical Education/Health and a Minor in Psychology. He hoped to continue his athletic career, but life had a funny way of changing his plans. In 2007, she graduated with a Master’s Degree in Acupuncture and Oriental Medicine and has been practicing ever since.

Jennifer uses many styles and techniques of acupuncture, but her main focus is Traditional Chinese Medicine including: Chinese Acupuncture Theory, Herbs, QuiGong, TaiChi, Shiatsu, and Tuina.
